Membership registration form is something which allows you to add registration fields as per your requirement, When you are creating a Membership plan you need to choose a regiration form. This form appears at the front end to on a Page or On the Popup (If you choose Plan button shortcode) to allow customers to register for the Membership.
Important: A single Membership regiration form can be used with Multiple Membership Plan.1. To create a Membership registration form Go to the menu Membership Plans->Membership Forms and Click Create New Form.
2. A default form will appear with the standard Shopify registration fields, You can add new fields simply by Dragging and Dropping elements available on the right side.
3. You can rename the field label as per your need, also if want to change it to a non-English language then you do it from here.
4. You can also add any additional field (Non-standard Shopify registration fields) to the Membership registration form. Submitted data of these non-standard fields will be mapped with the Shopify customer addition fields, and you check those submitted values under the Shopify customer details. To do so, you simply need to add the -cnote at the end of the filed name, like below.
5. After finalizing the fields, Next you can set the Appearance tab/setting you can enable Google Captcha to prevent the bots, Set the form with, button color, and set the thank you message as per your need.
